New Patient Appointment

Please arrive at least 20 minutes prior to your first appointment time.

The following should be brought with you to your first appointment:

  • Current Insurance Card(s)
    • Payments vary depending on your insurance plan.  Bring insurance co-payment, deductible or payment in full if you are not covered by insurance.  Payments will be collected when you check in.  Your insurance cards will also be scanned into your medical record.
  • Valid Photo ID
    • For your protection, patients need to provide a valid photo ID when checking in for appointments.  Your ID will be scanned into your medical record.
  • Medicines That You Are Currently Taking
    • Bring all of the medicines that you are taking in their original bottles so your physician can verify which medications and dosages you are on.  This includes any over the counter medications or herbal supplements.
  • Medical Records
    • Prior to your first appointment to establish, verify that your medical records are being mailed or faxed from your previous doctor.  This will help your new physician coordinate your care.  If you have not already done so, you can complete a medical records release form at your initial visit.

Existing Patient Appointment

Please arrive at least 15 minutes prior to your appointment time.

The following should be brought with you to every appointment:

  • Current insurance card(s)
  • Valid photo ID
  • Bring insurance co-payment, deductible or payment in full if you are not covered by insurance.  Payments will be collected when you check in.  Your insurance cards will also be scanned into your medical record.
  • All medicines that you are currently taking

Prescription Refills

Please do not wait until you are completely out of medication to request a refill.

You may contact your pharmacy or call the doctor’s office for prescription refills. Once you have called your pharmacy, the pharmacist will contact your physician’s office to ensure that your refill is appropriate. Federal and state laws do not allow physicians to call in prescriptions for most narcotics and other controlled substances.
